Tiffany founded 7TH & Madison Cares Inc. (7TH & Madison) in 2010.
Tiffany T. Weems embodies strength and perseverance. At age 14, she was an at-risk youth, and became a teenage mother, born and raised in Newark, New Jersey, and is now one of the most successful entrepreneurs, in her home state of New Jersey. Inspired by her fiancé, Tiffany is now the Founder, President and CEO of 7TH & Madison Extensions of Empowerment, a For-Profit organization, providing supportive services to at-risk youth, seniors, and adults with developmental disabilities in New Jersey. She opened the first Department of Human Services, Division of Developmental Disabilities (DDD) approved, Support Coordination Agency in Newark, New Jersey at the age of 31, making her one of the youngest Founder & CEOs in the industry at the time. Due to the success of the Support Coordination Division along with her passion for helping her members, she continues to serve as a Support Coordinator Supervisor to the families that have been with the organization since they opened their doors in 2015.
In early 2010, Tiffany established 7TH & Madison Cares Inc. for the purpose of providing services for at risk youth in Newark, New Jersey. Due to the lack of social capital, knowledge about the non-profit sector and a failed attempts to funding through local government contracts, Tiffany felt defeated. However, Tiffany refused to stop, instead focused on education, attended Essex County College and obtained her Associate’s Degree with the support of her fiancé, which gave birth to 7TH & Madison Extensions of Empowerment.
7TH & Madison Extensions of Empowerment (7TH & Madison) Support Coordination Division was founded in 2015 in Newark, New Jersey with the financial support and encouragement of her fiancé to provide Support Coordination Services to adults with developmental disabilities in need of intensive care management services. The name 7TH & Madison represents the streets in the city of Newark, New Jersey where they were both born and raised. Her fiancé was born and raised on Seventh Avenue, the North Ward and Tiffany was born and raised on Madison Avenue (near Clinton Avenue) in the South Ward of Newark. 7TH & Madison was initially operated out of their home at the kitchen table for a short time until eventually moving into a small office. The organization continued to grow and flourish for the next 9 years into what it is today.
In 2015, Tiffany established a partnership with Rutgers University, to have interns work with her one-on one to assists students in gaining relevant experiences in the field while continuing to develop and expand 7TH & Madison into other areas within the community.
Tiffany received her Bachelor of Arts Degree from Rutgers, the State University of New Jersey, School of Public Affairs and Administration, majoring in Public & Non-Profit Administration, and minoring in Psychology. She is a Ronald E. McNair Scholar and was awarded a scholarship and obtained research experience by completing the Ronald E. McNair Post-Baccalaureate Research Program, a 7-week intensive program is designed for scholars who want to pursue a doctoral degree. Tiffany gained comprehension and application of numerous research methodologies through working with a highly skilled staff and faculty mentor. With their assistance, Tiffany developed a literature review and 25-page in-depth research proposal.
After graduating from Rutgers, the State University of New Jersey, with a Bachelor of Arts Degree from the School of Public Affairs and Administration, majoring in Public & Nonprofit Administration, and minoring in Psychology, Tiffany identified her passion in May 2015 and founded the first Division of Developmental Disabilities (DDD) approved Support Coordination Agency in Newark, New Jersey. Her first position with 7TH & Madison Extensions of Empowerment was as a Support Coordinator Supervisor, providing oversight of all Support Coordinators within the organization, ensuring the organization's adherence to all Division of Developmental Disability policies and procedures as outlined by the Department of Human Services. While utilizing national best practices to administer social services that assist members in gaining access to needed program and State plan services, as well as needed medical, social, educational, and other services. Tiffany was responsible for ensuring the Individualized Service Plan was accurately developed and completed with the participant, their family, the Support Coordinator, and other team members designated by the participant. Tiffany was also responsible for verifying and reviewing ongoing monitoring of the provision of services included in the Individualized Service Plan. Tiffany also developed and conducted employee and member training, as well as mentored at-risk youth within her community.
In 2016, Tiffany administered all aspects of 7TH & Madison programs supporting people with developmental disabilities, at-risk youth, and seniors. She oversaw staff development and training and continued to maintain compliance with agency policies and contractual obligations. Tiffany also oversaw all agency quality assurance functions. Tiffany is responsible for opening a day habilitation program (day program) in Essex County, New Jersey for adults with developmental disabilities. Tiffany has also developed a Division for Mental Health Services within 7TH & Madison, which includes Intensive In Community (IIC) services, specialized tutoring and mentoring, career planning, prevocational training, supported employment, respite, community-based supports, summer camp, along with 12 additional supportive services. Tiffany is also philanthropically involved in the communities where she does business. She sponsors events for the local police departments, Autism Awareness Events, and non-profit organizations.
In 2019, while administering all aspects of 7TH & Madison Cares Inc. programs, which includes supporting seniors, at-risk youth, and individuals with developmental disabilities, Tiffany facilitated an agreement with NJ Transit, Access Link Division to administer assessments to participants. During this time, 7TH & Madison Cares Inc. supportive service programs expanded from 12 to 14.
Tiffany adheres to nationwide best practices to administer social services with full-time and part-time employees and manage budgets of more than $4.5 million annually. A focused leader with proven ability to transform, improve, and optimize global, digitally enabled support operations across diverse verticals; experienced in leading support, success, professional services, premium, renewal, and member-focused driven teams. Led direct reports to manage 80 cases statewide in account resolution, info management and governance, and budget modernization for over 5.5M annually. Oversaw 23 customer care staff, 2 outsourced engineers, and a $10.5M+ budget across statewide support centers to deliver exceptional support functions; created and led customer-driven programs. Recruited, trained, and led a new 25-person member success team from the ground up to drive 60% of the company's total flagship product line revenue while ensuring efficient, and effective member support. Tiffany developed and implemented short- and long-range plans and continued to oversee the expansion of programs, including the development of new programs and activities. She developed and operationalized the 7TH & Madison Extensions of Empowerment statewide Support Coordination program and continued developing and maintaining close working relationships with members, families, legislative leaders, and community organizations.
Tiffany has obtained affiliation agreements with various entities for many years as well as maintain active membership on several boards and committees that she is most passionate about that include but is not limited to: Alliance for the Betterment of Citizens with Disabilities (ABCD), The African American Chamber of Commerce of New Jersey, Supportive Housing Association of New Jersey, and New Jersey American Association on Intellectual and Developmental Disabilities.
Despite Tiffany’s numerous successes, her proudest achievement is being a mother to her son, a Licensed Social Worker (LSW) on the path to becoming a Licensed Clinical Social Worker (LCSW).
